Updated CSB Accidental Release Reporting Information…

In August 2020, the United States Chemical Safety Board (CSB) published requirements for submissions of reports on accident releases.

The instructions and form have been updated and re-released in June 2021.

If a site has an accidental release that requires submission of a report to the CSB , the site should be using the latest versions (listed in the file names as “revised 2021.02.19”) of the instructions and the form :

Look at the instructions and form carefully.

The order of some items from the previous to the latest form have changed (e.g., Items f1 and f2 have switched position).  The terminology of some items have changed (e.g., current Item f1 instruction is different than for the previous Item f2).  There is an additional requirement for a description of the accidental release (Item e).

It would be prudent to ensure that each site has the correct and most up-to-date versions of these forms to allow for timely submission of reports of accidental releases to the Chemical Safety Board.
